i am on klonopin 1 mg and slowly titrating down, just went down 2 days ago from 0.375 mg to 0.25 mg. the 0.25 mg pills are the odt version. i am having terrible panic attacks- neck tightness/ pins/ needles / shaky, could this occur from that change?

Need to examine: It is not feasible to provide a meaningful opinion without additional history, physical examination and may be some tests.  What you described could be due tonsillar infection and may require antibiotic treatment. Try gargling with warm salt solution while waiting to see your doctor.

Pain: Generalized muscle pain in shoulder & neck region could be from muscle strain due to poor posture, injury or even stress etc. Pins & needles sensation is worrisome for pinched nerve (possible disk herniation or ruptured disk) or peripheral neuropathy due to diabetes or vitamin deficiency among other causes. That's why it's so important to go see your family doctor for thorough evaluation.

Cervical Disc: Your symptoms of paresthesias in the left arm with neck movement are commonly seen with a cervical disc herniation. Oral steroids may be of benefit to reduce any inflammation of the nerve but an MRI scan would be warranted if the symptoms are not improving or worsening. Consider seeing a neurosurgeon and, at your age, one who is interested in arthroplasty for motion preservation.

See a PMR or ortho: Nerves from the neck travel to the arms, and stretching or pinching these nerves often produce these symptoms. Muscle spasm can arise from malposition of cervical vertebra, rapid stretch, or trauma, causing further symptoms. Pillows make a huge difference in providing support for the head over neck.  EMG studies can tell if a pinch is present.

CervicogenicHeadache: Most headache disorders are accompanied occasionally, and sometimes frequently, by neck pain.
