Early indicators: Almost any behavioral change can be early indicator. Some of the more sensitive and thus earliest include insomnia, social withdrawal, irritability, change in appetite. Previous decompensation pattern is also a good predictor for future decompensation indicators, in a given person.

It depends: This depends on the hospital, but most psychiatric units have treatment teams taking care of you. These include psychiatrists, nurses, social workers, psychologists, occupational therapists, internists, etc.  There are scheduled group therapies, plus your individual sessions w/doctors. Medications may be prescribed as needed.  It's a place for thorough evaluation, & for helpful treatment to start.

Shut ins: Or persons who have difficulty getting to treatment programs or appointments.  
Persons who have trouble taking their medications appropriately or who need frequent bloodwork done.
People who need regular injections.
Persons with agoraphobia who are terrified to leave their homes.

Trust: Yes. For a healthy adaptation to life with a family, in a community, and at work it is necessary to have a basic amount of trust, or you will never end up getting close to anyone. Many people have been burned repeatedly by individuals who did not deserve their trust. Not trusting anyone is a maladaptive response that won't really help. Counseling can help you decide who deserves your trust.

Accepted Answer

"Working" diagnosis: Maybe a "working" diagnosis could come out of an abbreviated session like that -- but this would not allow a complete study of past, social, family, or general medical history.  A working diagnosis means a basis for starting to work together, and it can be amended or appended as more knowledge comes forth. Being restricted to only 15 minutes' time would likely frustrate patient & doctor both.
