Nape pain.: This is either cervicalgia (pain in the neck) or  pain in the upper muscles of the neck and back including the superior trapezius and the splenius capitis.  Nape pain seems to cover them both.

Need more details : I need more details and information. It may suggest many things from minor to potentially major things. Could be a rash, neck muscle sprain or strain, spine or vertebral damage, tension headaches, infection, or some sort of neuropathy/nerve problem. Will need more details or other related complains.

Can't tell: Pus-filled infected areas that are just on the back of the neck are often mild staph infections rather than acne. If they've been around more than two weeks, a physician's office visit is in order, and allow any hair that's located there to continue to grow.

Neck pain can be due: To arthritis in cervical spine, nerve compression, muscle spasm in neck, upper back (trapezius), shoulders. Less common: infection (meningitis; sinusitis), tumor. See doc for evaluation, possible radiographs/mri, treatment.

Muscle spasms: The most common cause for this sensation is muscle spasms. Try warm compresses and gentle stretches. Do the warm compresses first then gently stretch the neck to one side and hold the stretch for one minute. Repeat for the other side. An anti-inflammatory may also help if you can take those (ibuprofen). If it is not resolving in a week or so see your doctor.

Cause of Neck Lump: Causes of lumps on the back of then neck or nape are several. The most common cause  i see are sebaceous cysts. Also lymph nodes related to local inflammation like acne, insect bites and less likely, cancer. Any persistent new lump or one that is growing should be examined by a physician in case it reflects a more serious underlying disease.

Eval: This can't be diagnosed with the information that is available. A history, physical examination &amp; other studies may be needed to determine cause/causes. Once accurately assessed a treatment plan can be developed.

Anatomic landmark...: This is probably what is called the vertebrae prominens, or 7th cervical vertebra (C7) and has a distinctive long and prominent spinous process that is easily visible in this area.  It's normal.

Hard to know, but...: The GI tract has a very limited # of symptoms to signal its distress--nausea/vomiting, diarrhea/constipation, bloating, pain, fullness, bleeding, etc. As you know, many of these characterize ibs, but are also seen with inflammatory, infectious, ischemic, malabsorptive, and functional disorders. Directed lab work, imaging, biopsies, stool studies are appropriate in excluding non-ibs pathology.
