Truama or fungal nai: Discoloration may be due to fungal nail changes causing excessive sublingual debris vs trauma induced. For example certain activities may cause bleeding under the nail or dry blood. If in the tip and drains out it causes automatic desertion of the nail. If no sorrow ding redness or swelling it's not urgent but should be looked at. Likely cause usually fungal nail or trauma

Several possibilitie: If you are an athlete you may be pushing your foot against the front of your sneaker and that is causing your nail to come off. Another possibility is that your shoes are just too small. I would go to the shoe store and have your foot measured to be sure you are wearing the right size.

Needs to be seen: That should be evaluated by a ER physician/hand surgeon.  If the nail isn't secured back down underneath the fold of skin at the base of the nail it can cause healing issues
