pain in my left hip, buttock, thigh. knee and down my legs.thigh burns feels like

Return of symptoms?: Are those where your symptoms were existing prior to the injection?  If so, seems like the injection may have worn off.  If not, need to step back an evaluate these symptoms as if they are new.  If pains is SEVERE and walking is limited, please have your physician evaluate you and attempt to find the source of the symptoms!

Sciatica: Even though you have had surgery in the past the solution for the sciatica would still most likely be achieved by conservative means. You should be re-evaluated and discover cause.  Your prior history makes sciatica more likely, but some other etiology that what lead to your prior surgery possible. Physical therapy still the best bet for recovery.

Trauma: You suffered a mechanical trauma to your back and your symptoms are suggesting a pinched nerve due to inflammation. You should seek immediate medical attention for 2 reasons, first symptom control as pain is unbearable and second to have imaging and treatment as needed depending on findings and potential for further damage.

Get to the Hospital: If you have such severe pain you need to see a doctor immediately. if you do not have a doctor go to an Emergency Room of any hospital where you can get in. This pain can be caused by more than one serious conditions in your abdomen which may or may not be liver related. it is urgent to see a physician today.

Inflam. Arthritis?: Have you seen an rheumatologist? With multiple joint complaints, it is reasonable to be worked up for an inflammatory condition similar to rheumatoid arthritis. These conditions do not necessarily show up on ct or MRI scans.

Neurology consult.: I would recommend a neurology consultation. Leg/thigh pain can have different causes. A nerve conduction test or other tests may help pinpoint the nerve involved. A normal lumbar spine MRI may not completely rule out nerve problems as sometimes a pelvic or leg MRI or even an MRI neurography (which is not commonly done) may be needed to study the involved nerves.

Knee or back?: You need to consider if it is a primary knee problem or a back/disc problem with radiation of the pain to the knee. I you have back pain with radiation into the thigh that stops at the knee, you would see a neurosurgeon for possible nerve root impingement in the back. If the pain is just in the knee and in particular if you have had a history of injuries to the knee-see an orthopedic doctor.
