See specialists: Often times shoulder and neck pain can go together, meaning that you could not only have a pinched nerve in your neck, but also a legitimate shoulder problem.  Shoulder pain does not radiate below the elbow.  Any pain that you feel shoots down the arm to the fingers can be coming from the neck area.

Neck arthritis: There are several joints and intervertebral discs in the neck.  These can wear out over time leading to pain.  Degeneration of the joints or discs can cause neck pain, headaches, and/or shoulder pain.  Poor sitting posture can actually put more pressure on these structures increasing the pain.  Be sure to have a good sitting posture or lumbar support to reduce these symptoms.

ER vs primary care: From the accident you describe, most people would have been seen in the er afterwards.  If you were not and now have these complaints, go there now.  If emergency exams & xrays have already been done in an er and no life-threatening injuries found, see your primary md/do as soon as you can.  If specialist care like orthopedics or other is needed, s/he can facilitate those referrals.

Possibly: Almost impossible to say without an examination. If the occurrence of your neck pain diminishes with the use of the antibiotics and the resolution of the abscess, then they were related.

Muscle spasms : Even young people can start experiencing neck problems due to neck nerve root irritation.  This can occur because of lifting heavy things, sports injuries or other accidents.  Repetitive stress injuries can also initiate the symptoms, sleeping on the stomach  with the neck twisted to one side can kink the neck nerves.  Drinking cold liquids can initiate muscle spasms. Accessory Muscles: The diaphragm is the muscle that is used primarily for breathing. It is attached to the lower ribs and separates the chest from the belly. Other muscles used to breathe are called accessory muscles of respiration. They are in the neck and between the ribs. Anything that irritates those muscles can cause pain with breathing. Oddly, some irritations at or below the diaphragm can radiate up.

Visit your MD: Chest pain is a common entity. If you feel substorms like chest pain that lasts for over a minute, you should visit your doctor. You need a complete exam and you need to tell your doctor your medical history , family and social history , and medications. Good luck.
