PAT: I would be VERY concerned about a pulse of 230 bpm, even if regular, even if "generally fit."  People who feel no symptoms may have cardiovascular disease and suddenly drop dead.  See a cardiologist as soon as possible.

Maximum: Maximum predicted heart rate is calculated as 220-age. For a 22 year old the maximum heart rate is therefore 198 bpm. If your heart rate reaches 250-270 during exercise, it would be best to get evaluated.

Exercise and heart r: The heart rate increase during exercise.  Many factors influence the degree of increase.  More fit people has less increase then non fit ones at the same level of exercise.  So my advise: do not worry about target HR just gradually increase your exercise level and duration and do not try to target the HR

If you're a seasoned athlete, it's due to improved stroke volume (blood ejected with each contraction) requiring a reduced heart rate to achieve a given cardiac output (the product of sv and hr). Other possibilities are: medications, vagal reactions, chronotropic incompetence, av nodal block, or sick sinus syndrome. Incidentally, typhoid fever can cause it too.
