Otitis externa or: External ear infection can be quite painful.  You need to see a doc, preferably, an ent, as soon as possible.

Need drops Rx: It sounds like you need to have that ear looked at.  A prescription for ear drops to help with the swelling, infection and pain.  You may need a wick inserted for a few days to help the medicine get into the swollen ear canal.

Otitis externa: The fact that you tell me that his ear swells up makes me think this is an external canal infection, as opposed to a middle ear infection. This is caused by bacteria invading the external ear canal ( like a swimmers ear). He can try putting white vinegar in his ear once to twice daily for prevention of these infections.

Ear effusion?: Sounds like (no pun intended) that you have either developed a fluid collection behind your ear drum (middle ear effusion) or your eustachian tube on that side has become blocked.  Best to see medical provider for evaluation.

Middle ear : infection will hurt more in the back of the ear wile an infected ear canal will hurt more when pressing on the Pina, the little bump in front of the ear. A middle ear infection can also be associated with eye infections. Most of the time these are viral and resolve on their own.
