i was told that my mirena (levonorgestrel) is sideways in my uterus...can this be causing my lower back pain? and could it affect my bowels as far as being constipate

Needs removal: If your iud is not in the correct position, it will likely cause pain and cramping that can be felt as low back pain.  This iud needs to be removed!

Fatty liver: chronic condition associated with obesity and diabetes, can cause cirrhosis over time.
Fibroid uterus: common in 40 year old women
kidney concretion: I have no idea but maybe this is calcium deposited in the kidney.
No 1 diagnosis puts all of these together.  You need to see your doctor.

? endometriosis: Black stools are usually a sign of blood in the bowel. If the lower bowel bleeds, then red will be seen in or on the stool. Black stools usually mena that the upper digestive tract has blood, as in stomach or upper bowel. Either way black stools can be GI bleed and can be serious and needs to be checked.

Start with Gyn: Since your pain is associated with your menstrual cycle this may indicate endometriosis. The best place to start is with your gynecologist. They can begin the evaluation. Since your digestive tract is involved they may want to consult a gastroenterologist. If they are considering laparoscopic surgery they may prefer a colorectal surgeon in cases endometriosis is on the bowel
