Common enough: Penile erection in any male-esp prolonged erection, called priapism-can be very uncomfortable and painful. Common but not as painful in small children.Try to get child to relax by being closely held for comfort and trying to get child's attention on a toy or something else.Also try not to stimulate penis while cleaning him and while changing clothes. Sometimes use of warm water locally may help.

: In infants with Eczema, Diaper rash can quickly become superinfected with yeast. Apply an over the counter cream containing clotrimazole or miconazole to treat yeast infection, then use a thick diaper ointment daily to provide a barrier against irritating feces and urine, avoid commercial baby wipes- use only water, and change the diapers more frequently.

my 4 month old's penis is swollen and red around the foreskin. he's circumcised. we wipe it with every diaper change. no pain. what could this be?

Skin sticking: I suspect that you have not been shown how to retract the skin on the shaft properly every now and then and clean that area. The skin then tends to stick and the secretions by the glans then irritate because they get trapped under that skin that is sticking to the glans. You may even have to be a little forceful to clean out that trapped smegma to get it to heal correctly and avoid recurrent probs.

Dont need to retract: It is not necessary to retract the foreskin in a 1yo. If there is redness or irritation, it may mean that there is soap left behind after a bath or that there is an infection. See your pediatrician for an evaluation to determine if there is an infection present and how to care for your son's penis.

?Frenulum tear: Frenulum is web on underside of urinary opening, connecting penile head to shaft. Can tear. even when circumcised, and cause some burning with urination. Frenulum is usually left intact when babies are circumcised. Son may have torn it pulling penis out to urinate. Should settle on its own. Frenulum is soemtimes called "male hymen" because it can tear during initial attempts at vaginal intercoarse

Hard to guess: The pediatrician or family doctor who takes care of a toddler with irritated foreskin can advise on what to do. An exam is usually needed. If there is no discharge, sometimes some mild anti-yeast cream and some mild anti-bacteria cream can clear things up. The doctor might ask the parent to bring the cream and child in, to show how to best apply the cream. Sometimes a video consultation suffices.

Most of the time the tear or abrasion will heal rapidly within a few days. During this time you will be much more susceptible to STD infections so use a condom.

Few possibilities: Inflammation of the penis can be related to conditions such as balanitis (swelling of the head of the penis) balanoposthitis (inflammation of foreskin and head of penis in uncircumcised males). It also can be due to urethral irritation or infection. Redness and/or swelling of the penis needs examination. Further evaluation by a physician is needed.

Your : Your son definitely has a problem.  He may have a condition called lichen sclerosis.  He should see a urologist or pediatric urologist for evaluation.  The problem may be treated with steroid cream.  If severe, he may require a circumcision.
