Not at 35 years old: sometimes studies answer questions and present new ones.   The Chiari malformation may cause problems but for a 35 unlikely at this phase in your life.  Most importantly for you it has nothing to do with tinnitus.  Why are you taking labetalol?

5mm: is a borderline measurement.  There is some variability between investigators regarding the precise range of normal. In the abscence of any other findings, most authors would consider 5mm at the upper limit of normal.  Some would term this "benign tonsillar ectopia", an incidental and asymptomatic finding. Throat/ears: Often throat pain or infection can be referred to his ears. Have him examined and checked for tonsillitis or a throat infection. Sometimes the adenoids in the back of the nasal passages can also be infected and cause ear pain. All the best.

Basically,: Chiairi malformations are characterized by herniation of the cerebellar tonsils through the foramen magnum into the cervical spinal canal. Depending on the type of malformation there may be other associated findings. The graphic shows the changes at the level of the skull base.

Chiari Malformation: Chiari malformation shows up on a MRI as an extension of the cerebellar tonsils (the lower part of the cerebellum) into the foramen magnum, with or without involving the brain stem. Usually stable: Chiari malformation can be minimal and asymptomatic throughout life, incidentally found on an MRI done for another reason, or it can be significant and require surgery.   It usually remains stable in each individual.
