If there is any doubt further testing can be done for  the Epstein-Barr virus and the presence of present or past infection, generally. In the acute phase of mono there may false negatives with either method.

Same antibodies: The simple screening test for mono often just  indicates the presence of antibodies to the EB virus which should be present for quite a while. Viral detection studies could be ordered but these are more expensive &amp; generally reserved for confusing cases. Specific  IgM antibody to the EB virus is more specific to new infection but this fades leaving the IgG antibody for long term

Screen vs specific: Monospot is a non-specific quick screening test but ebv ab testing is most specific and tests against the rapid response to the capsular antigen and the nuclear ag.

Unlikely: The label "Mononucleosis"  was minted back before we had expanded testing abilities to find the germ or antibodies that turn the mono test positive. Its label came from the high level of monocytes (cousin of lymphocytes) found on a CBC. There are lots of medical &amp; emotional conditions that can cause symptoms which overlap with that of mono.
