Probably not: When the body fights back at mono the lymph nodes in your neck swell to produce antibodies. The tissue of the throat will need time to heal. Your symptoms sound more like a new irritation - body responding by coating area with phlegm and the lymph node responding with antibodies. Gargle warm salt water and rest. Common problem: Viruses are a major cause of sore throats and they tend to self resolve over a 1-2 week period.Mono and strep have far more associated problems than the others but once these are excluded,comfort care is the primary treatment.

Maybe: Difficult to be sure without seeing the rash.  Certainly many varieties of rash may occur with mono and other viruses as well.  Could be a drug eruption as well. The description suggests the need for an exam by your doctor.

Virus.: Many viral illnesses (common cold) cause symptoms similar to strep or mono.  Sometimes acid reflux can also cause a raw or painful throat.

: Might be infectious or contagious. A recurrence of throat pain can be from a strep throat infection (antibiotics are needed), a cold or influenza (antibiotics usually not used), mono, COVID-19, allergies or irritants, etc. Usually, a person sees the doctor if the sore throat is bothersome or doesn't go away quickly. One should not simply assume the cause is mono, as it could be a different cause.

Infectious mononucle: How long  did he have it? Did his doctor mention  anything about enlarged spleen?  It is important  to get this information before he can go back to workout or be with his girlfriend. You might as well talk to his Dr  about your concerns. Good luck.
