Carotid doppler: Your carotid arteries were heavily enough calcified that the calcifications were blocking the ultrasound beam enough that the degree of carotid stenosis cannot be evaluated, thus the suggestion for ct angio. Heavy calcification may also effect the ct angio too. Mra (magnetic resonance angiography) would be another consideration which would not be effected by the calcium.

Statins: Statins have been proven to help prevent strokes. Specifically they have been shown to stabilize plaque and in isolated cases may have reversed plaque burden.

What does this term : It is simply a description of what the person reading the scan thought the area of the diseased vessel looked like.  The amount of blockage is usually the most useful feature of the report with the description being of secondary importance.

Maybe: The nuclear stress is about 90% accurate. LBBB can create "false positive" where the test is abnormal and the patient is OK. Other artifacts can also cause a test to be abnormal if the patient is OK. The coronary angiogram is the only way to be sure. The options are cath or CTA. It sounds like your physician has elected to proceed with cath, which sounds reasonable.

Be diligent: Sounds like you are in good shape, so keep up the good work.  Your Nuclear study as i see it in your question-shows Moderate to Severe ischemia so that may be a clue to your Painful respirations, etc.  A cardiac Cath would be definitive for plaque/arterial pathology. Continue with the Cardiologist as maybe medication can be used to treat you. Bet wishes.

Yes: The tannins and other agents in wine tea and coffee are fully capable of causing these stains. Of course the solution is proper plaque removal with brushing and inter-dental cleaning to get rid of that stained plaque. If it's there long enough to acquire staining, it's been there too long!

Tests for plaque: The degree of plaque formation can be estimated by coronary angiogram via cardiac catheterization, duplex ultrasound for carotid and peripheral arteries, intravascular ultrasound, ct scan or mri.

Topical therapy: Treatment depends on the size and number of the placques.  One or a few can be treated with topical agents to decrease the thickness of the placque and treat the inflammation.  Topical steroids and vitamin d is a current successful combination.  Intralesional steroids or topical steroids under occlusion may also be used.
