Cirrhosis: My policy is to never remove hope from the situation, no matter how dire things seem to be.  There is effective treatment for hep c and there are mitigating therapies for end-stage liver disease, including transplantation (although do not know if there is an age limit for this). In any event, predicting death is not a doctor's purview, and advise you to make the most of whatever live gives you.

Any time: If this person is not a candidate for a portocaval shunt, a liver transplant, or treatment of the underlying liver disease (and don't think this is always impossible), death may come at any time from variceal bleeding, pneumonia, or some other mechanism. Usually these people do not survive more than a few months but there is no predicting the exact time.

Talk to the: Surgeon and anesthesiologist, frequently that surgery can be performed under local anesthesia with sedation.

No reputable: medical practitioner recommends milk thistle for someone with liver disease. Don't believe everything you hear.

Mlkthistle=Silymarin: Milk thistle seed extract / silymarin is a natural product with antoxidant/ antifibrotic activity which may decrease hepatic injury & scarring.  Unfortunately, 14 studies found that while liver enzymes improved with silymarin,  no clear evidence of reduced mortality, improved liver tissue on biopsy, or markers of liver function in patients with liver disease. It does devr hcv replication. No harm.

Hepatologist: Instead of trying to manage your condition yourself, go and see a liver specialist and follow their directions.

Possibly: There have been studies demonstrating benefit of milk thistle in a variety of liver problems, possibly in part due to increasing the concentration of glutathione in the liver.  Glutathione is used to neutralize toxins.

No: If you have a fatty liver or liver disease stop eating fatty food, stop eating fried food, stop drinking alcohol, stop taking pain pills and lower your cholesterol.  Those type of actions help the liver.  Discuss the different medications you take with your doctor to make sure they are not causing any interactions that can harm the liver.

Doubtful: According to nih, "previous laboratory studies suggested that milk thistle may benefit the liver by protecting & promoting the growth of liver cells, fighting oxidation (a chemical process that can damage cells), & inhibiting inflammation. However, results from small clinical trials of milk thistle for liver diseases have been mixed, & two rigorously designed studies found no benefit.".

with non-alcoholic fatty liver is milk thistle and vitamin e+ exercise effective and if so what dosage?

Unknown: as to whether they are effective. There are studies on both sides of this issue, and the majority of studies have not shown efficacy of either medication in preventing the progression or promoting the improvement of liver inflammation.  This is particularly true with vitamin E, where all studies have shown lack of efficacy. A few studies of milk thistle have shown small improvements, but most not.
