Diagnosis?: This sounds like a reading from an MRI and not really a clinical diagnosis.  Depending on many factors including whether you had an injury, etc. Treatment may include a steroid injection into the subdeltoid bursa and physical therapy.

PT and medication.: These conditions are part of impingement syndrome and represent inflammation in the tissues surrounding the rotator cuff.  Rest, ice and nsaid's are first line treatments, followed by therapy and oral/injectable steroids.  If these measures fail, then shoulder arthroscopy is performed to remove the inflamed bursal tissue and bone spurs. The rotator cuff is evaluated and if torn, repaired/debrided.

The "SPACE": the top of the shoulder needs to move freely is too small since the bursitis and the bony spur BOTH IMPINGE on that space. This is called "Impingement Syndrome"  Steroid injections might give some relief but the ultimate treatment is surgery (Called "Subacromial Decompression"

Impingement: The space between the palpable shoulder bone (acromion) and the all-important rotator cuff, that helps raise one's arm above shoulder level, contains a small bursa or shock absorber between the undersurface of the bone and the overlying surface of the rotator cuff. Once it gets inflamed and angry, it can become very painful and fills with yet more fluid. Injections are usually tried initally.

Therapy program: Most of the time subacromial bursitis will resolve with a course of physical therapy. If it doesn't then a cortisone injection may help. If all else fails, arthroscopic surgery may be indicated. See an orthopaedist to confirm the diagnosis and guide treatment.

Inflamed bursa: A bursa is a small sac that often cushions an area around a bony prominence and also often sits next to tendon. Subacromial bursitis often accompanies a significant rotator cuff tear and less commonly, can occur by itself as a result of inflammation. Treatment can include rest from aggravating activities, anti-inflammatory meds, physical therapy and corticosteroid injections.

Avoid side sleeping: Avoidance of side sleeping or stomach sleeping with arms up under the pillow will be less provocative and help lessen the discomfort from subacromial impingement. Back sleeping is an excellent option.

Orthopedics : Recommend consulting an orthopedic surgeon  before embarking on any more cortisone injections which have not helped you to begin with.

Injection: Have you had cortisone injections into the bursa.  These help tremendously well in my patients.  If the relief doesn't last long, you may want to see a physiatrist or orthopedic surgeon that specializes in the shoulder to get a better workup (likely with an MRI or possibly shoulder arthrogram).
