Headaches are : On a continuum not nessesarily discrete events. After a car accident it is possible to have severe musculoskeletal driven headaches which can trigger and or progress to the vasospastic events known generally as migraines. If unsure get a second opinion. A neurologist who specializes in migraines or the like.

Dilated pupil: You should see an ophthalmologist for complete eye exam. Unilateral pupil dilation and be nothing at all or something serious.
So, see your ophthalmologist.

Many reasons: You could have chronic daily headaches, you could possess tension or migraine, you might have intracranial issues, but most likely you have benign problems which need control.  Lots of approaches are available including meds, botox, even the recent "migraine surgery".

Get it evaluated: If you have true migraines you need more specific therapy. See a neurologist for better headache control.

Migraines: intractable head pain needs to be fully worked up before a decision is made that it is just classic migraines.  Have they done a full work up?

Headache (HA) help: Your list shows aspirin, ibu, and tramadol; if that's all you've tried, don't worry.  These are painkillers, but they don't 'break' a ha.  I usually prescribe Prednisone for intractable ha.  If that doesn't work, i sometimes admit people to the hospital for dhe.  Usually takes about 2-3 days to work and we can give other IV medications at the same time.  You may need to take a daily med to prevent.

why is aimovig migraine medication a shot? why is it not oral (would it not work if it were in an oral form)? some people don't like to get shots.

Most effective : delivery system: if you don't want injections, ask for an alternative oral medication.
