Supplements may help: MTHFR (methylene  tetrahydrofolate (folic acid) reductase) deficiency is a genetic condition in which elevated homocysteine is due to abnormal folate (folic acid) metabolism. Elevated homocysteine has been implicated w/ cardiovascular problems &amp; other medical issues. Because its a genetic condition, there's no cure.Often times supplements (MORS) which use an alternative folate (folic acid) metabolic pathway may help the condition.

YES.  You should: also take methylated B vitamins.  Even with patients who are homozygous for this trait, there's variability in the ability to methylate folate &amp; B vitamins.  As such, there's no one recommended dosage.  Lab tests may help with dose adjustments. Do keep in mind that you should NOT take plain OTC folic acid.

Genetic mutation: Methylenetetrahydrofolate reductase (mthfr) is the rate-limiting enzyme in the methyl cycle, and it is encoded by the mthfr gene.   

Genetic variation in this gene influences susceptibility to occlusive vascular disease, colon cancer, leukemia and neural tube defects.

I am not aware of any relationship between mthfr levels and problems with epidural punctures.

An aggressive lab is promoting testing widespread testing to see which of several common alleles people have at this locus. It's not clear to me that there's any real public or individual health benefit in this testing. All folks should get plenty of folate (folic acid), B12 & b6.

It is very common, but your provider, with your specific test results, can provide help.

No: Look for another cause. 30% of white people have one dose and 5% have two. There are some correlations found by some groups but most people in the pathology community do not believe this is sufficient to explain clinical illness. I also see that you are on Methotrexate which can raise mcv, and an elevated B12 means nothing.

MTHFR: Avoid unmethylated forms of vitamin B12 and folate (folic acid).  Instead, use methylcobalamin and methylfolate (folic acid).  Also, eat dark leafy greens, uncooked, twice a day.

Consult: I'm not familiar with the acronym PAI so you might want to rephrase your question describing that condition. I'm assuming it might have something to do with platelet problems. MTHFR used to indicate the need for Aspirin daily during conception and pregnancy. But that recommendation has been rescinded. Talk to a perinatologist for a definitive answer regarding PAI
