Ibuprofen: Is the best medication for menstrual pain. Make sure you take enough, 600 mg is recommended.  All brands are equal, generic, motrin, advil (ibuprofen).  They are all the same ingredient.

Heating pad: It's worth a shot to apply to lower back region.  Get some Aspirin, Tylenol, (acetaminophen) Midol, Motrin, etc...as soon as you can.

Menstrual pain ?'s: Sever pain with menstruation usually comes from endometriosis, which is spread of endometrial tissue through the lower abdomen and large intestine.  Oral bcp's will diminish the pain, and arrest the increase in endometriosis if taken continuously for several packs. These "implants" of endometrial tissue inside the abdomen cause bleeding which is a strong irritatant intraperitoneally= pain.

Period pain: Menstruation when endometriosis is present abdominally can cause referred pain due to the inflammation caused with blood inside the abdominal cavity, located on sensory nerve endings on the round ligament and other structures.  If the "hip pain" is caused by endometriosis,  bcp's can reduce the pain considerably by shutting down the ovaries .

Pain: If i read your question right you have irregular menses and then you have pain in center of chest and then to shoulder blades.  The irregular periods are a sign of hormonal issues.  I would be suspicious of thyroid problem. As to your specific pain substernal pain should always be checked for possible heart problem.  However, i would be highly suspicious this could be gallbladder.

Probably: It is premature to conclude that the myomectomy did not improve your period. The procedure will not always improve painful and heavy periods but you will know better with your next cycle.

New onset pain,: fever, recent sexual intercourse, nausea/vomiting. These symptoms could be indicators for PID.

Heavy clotty menses: Normal menstrual periods do not contain large clots. Clotting is a function of rapid arteriole and not venous  blood loss consistent with hemorrhage. You need to contact and see a GYN MD ASAP to avoid anemia and determine the cause of the hemorrhage.
