Controlled Delivery: Diltiazem is available in many different forms and has several brand and generic names. The “CD” following diltiazem stands for Controlled Delivery, which has an extended duration of action. For example, Diltiazem CD only needs to be taken once a day, while other formulations may need to be taken twice daily.  
Diltiazem can be used to treat high blood pressure and abnormal heart rates and rhythms

Yes: Diltiazem interacts with Ranexa (ranolazine) in the liver and increases its level in the blood stream.
Can prolong qt and cause rythm disturbances.
Other side effects from renexa likely:  constipation, dizziness, headache, nausea., palpitations, dizziness and blurred vision.  These are more likely with this combination, but many people on this combination of drugs.
Double check with your doctor.

have avg of 20k pvcs a day all tests are normal (echo, etc). beta blockers don't work. ep wants to try diltiazem. how will this drug help with pvcs?

It's a CCB: Many channels for rhythm in heart. 
Beta blockers didn't work.
Next he is trying calcium channel blocker (diltiazem).
If this doesn't work there are many other drugs. But with increasing frequency of side effects. 
BB's and CCB's have safer side effect profile.

Diltiazem gel: It is used for anal fissure, but I am not sure that this formulation exist in the us.

Yes: Any anti arrhythmic or rate lowering medication can have the side effects that you described.  Talk to your electrophysiologist/cardiologist about it.  If you have paroxysmal atrial fibrillation, a "pill in the pocket" approach might work better for you.

my doctor just recently prescribed me a new high blood pressure medicine, diltiazem,  when i was speaking to his nurse i forgot to tell her that i had filled my prescription for viagra. i want to know if i can take those two medicines together.

Yes: It is safe to take diltiazem and Viagra together. Neither medication affects the other.
