Normal: Acth is supposed to be low if you are taking prednisone. It should bounce back once the drug is stopped.

Unlikely: Not sure what you mean by adrenal fatigue. If you have adrenal insufficiency this can be effectively managed with replacement therapy. See an endocrinologist. Mebendazole should not affect the adrenal.

Adrenal hyperplasia: The commonest type of congenital adrenal hyperplasia is caused by deficiency of 21 hydroxylase, classical type presents at birth, but non classical may present later in early childhood or during puberty.It is difficult to answer all about it, but some salt depleted patients need to be on steroids and the patints need to be monotored for other defects, it should be trated by a pediatric endocrinologist.

Evaluated by endo: hypoadrenalism either due to a problem with pituitary gland or the adrenal gland, can be easily diagnosed with appropriate hormone testing by a trained endocrinologist. It is highly important, to establish the diagnosis before starting the treatment, as wrong diagnosis has consequences. I would recommend referral to an endocrinologist for evaluation.

Adrenal Insufficient: There are herbal remedies, including ginseng, ashwaganda and rhodiola, and also dhea.

That: Is a tricky one because   there  is  not enough  evidence of low cortisol causing low levels of serotonin. Depression, chronic and severe, is associated with low serotonin and could lower cortisol levels.

Accepted Answer

Yes: adrenal suppression is dose related and can even occur at the replacement dose of 7.5 mg prednisone or equivalent. You have to take higher dose for stress situations ie infections bacterial or viral with increased dose of steroid.

is it risky to be on 0.05 mg florinef (fludrocortisone) ever other day for pots? i struggle with when off it. concern for bones, thinning skin, adrenal suppression

Non-issues: B"sd Florinef (fludrocortisone) is the only aldosterone like agent available. To my knowledge, unlike corticosteroids it does not suppress the pituitary adrenal axis or cause thinning of the skin.  If anything it should decrease renal calcium excretion and possibly decrease bone loss.  the main concerns; 
1.hypokalemia 
2. hypertension, especially when supine.

Currently there is: no cure for POTS. All treatments are palliative to avoid symptoms and complications from syncope. It is a molecular genetic abnormality of the autonomic nervous system. Although there is potential for long term side effects from the Florinef (fludrocortisone) the alternative is harm and disability from non treatment. Of course Sx come back without Florinef (fludrocortisone). Ask about other therapies that might be reasonable.
