It is made from: onion skin and can thin out the thicker scars, but  as a dermatologist i don't feel it really lightens the color of scars

It may: Mederma is made from onion extract and that has been shown to improve the appearance of scars. Silicone, pressure, and steroids have also been shown to help. Avoid sun exposure.

Never: A 7 year old scar is what you're going have permanently.   There is nothing available that can remove scars.  Mederma can minimize scarring, but will never completely erase a scar.

Depends on scar: Scar creams and gels work differently, hexilak has silicone, Mederma does not.  Benefit is questionable, however it does not hurt to try them. Most important, massage the scar twice a day and use sunblock over it. For breast and tummytuck scars i suggest biocorneum, for facial scars i would reccommend mederma. Hypertrophic, wide or keloid scars are handled differently. Consult your plastic surg.

Carrier material: The medermaa gel is mixed with a carrier. That remains on the skin surface and dries? That forms a thin layer . That is what one sees with the small flakes.

Not likely: Excision, revision or laser treatment might be the better alternatives.

See below: Most people don't need anything other than come cacao butter on the wound to help the healing. Prolonged use of neosporin will lead to problems.
Use of me derma and silicone sheeting may be helpful when called for.

Not likely: Note that mederma and the majority of topical treatment for scars have been shown to have no improvement on normal wound healing, and would be very unlikely to change the sensation (but it won't hurt.)  vitamin e and massage have some very week evidence of benefit. Protection from the sun is the thing that has shown the most benefit- use and reapply good sunblock  during the first year of healing.

Silicone: Mederma is less expensive and available, but works marginally in my experience. I prefer the silicone based products like biocorneum.

Laser: You may see the most improvement with laser therapy. Even with new wounds, mederma and most topical treatments for scars have been shown to have no improvement on normal wound healing, (but it won't hurt.)  vit e and massage have some very week evidence of benefit.
