Can be all those: All of us need to have some healthy self-interest -- healthy narcissism -- to deal with life.  But when this becomes greatly exaggerated,  squeezing out the needs of others, we get into difficulty.  There are narcissistic personality disorders which maybe 4-7% of our society has.  These involve fixed patterns of relating with several criteria.  Yes it is treatable - with great care and patience.

Talk: Personality Disorders aren't diseases - they are ways of relating in the World. Medications might calm you enough to learn new ways of doing that - but it's theraputic re-parenting/learning that might help you make changes. It's challenging and takes time, but a good fit w/ an experienced therapist can succeed. You'd discover how it developed and maybe decide to try new approaches. Trust's a must!

Folks don't like you: That you seem to care might mean you don't have it - at least not severely. It would mean that you feel very self-important and worth way more than others. Even if you are smarter, more attractive, etc than those around you, it makes relationships impossible if you're narcissistic. Pds are all hard to change, but if you really want to - good interpersonal indiv tx &/or group tx can really help.

May exploit others without concern for their needs.

Yes, BIG problem: A personality disorder is an enduring, pervasive, and inflexible pattern of inner experience and behavior that differs markedly from one's culture, begins in adolescence or early adulthood, and causes distress in important areas of life (relationships, work, etc). Narcissistic pd causes problems b/c they are grandiose about themselves, need constant admiration, and can't empathize with others.
