Tender gallbladder: When the sonographer presses directly over the gallbladder, and the patient expresses pain, more than when the sonographer presses anywhere else, this is said to be a positive sonographic murphy sign. Technologist has benefit of visualizing gall bladder while placing pressure on gall bladder. Mild murphy sign is probably mild pain with maneuver. Sometimes sign of cholecystitis, inflammation of gb.

Bright spots: Kidney stones show up on ultrasound as bright echogenic foci (white spots) within the substance of the kidney. If the stones are larger than 3 or 4 mm there will be a darker area behind the stone representing an acoustical shadow. Obstruction of the kidney by a stone will cause dilatation of the kidney's collecting system which will appear as black fluid spaces within the kidney.

Not always clear cut: Purely simple, smooth ovarian cysts are considered benign. Cysts with debris, septations or nodules along their walls can be concerning. Sometimes we do follow up ultrasound to see if they grow. If there is enough concern, MRI helps. Correlation with patient age, family history, blood tests and possibly biopsy also help us be sure.

Need observation: Because does not have  enough indication for a biopsy , some times will show very small area of abnormal tissue , may be  in one view only , if otherwise normal radiologist will advise the surgeon to follow closely and  repeat on that side in 6 months instead of waiting for the usual  1 year follow up study.

Hydrocephalus: The ultrasound is to exclude hydrocephalus, large ventricles in the brain.  However, most of the time a "big head" without neuro symptoms or development delay is just a big head.  Sometimes it can also be from having too much CSF (fluid) surrounding the brain.  It is common in infants, goes away as baby grows, and does no harm.

Need much more information and without that and seeing the US cannot help. Are you talking about inside your chest or in the breast for example. Get the report and get back with us but better yet ask your Dr. who ordered the test.  He or she knows you best.
