last year i was hiking and was bit multiple times by horseflies. 1 bite swelled very large on my foot and another had a "bullseye" looking rash. i've had many health issues since then that seem lyme disease related. who can i contact to get tested?

Your Family Doctor: You should consult your family doctor. If you don't have one,then go to a clinic. Horse flies bite will not cause Lyme Didease. But you can be checked by physical exam and blood test

See a neurologist: This is one situation in which i would urge a 2nd opinion with a neurologist if you have not already seen one.  If the neuropathy is related to lyme disease (and imo that is a large "if") the injury to the nerves heals very slowly. A neurologist would rule out other causes and offer you meds which are often very helpful in minimizing the symptoms.

Must treat: This is very suspicious for Lyme. About 30% will never test positive for Lyme. You need a course of antibiotics ASAP. Lyme must be treated quickly, or else it becomes chronic and is much harder to treat.

If you don't have...: ...The characteristic circular rash (erythema migrans), the only symptoms are often aches, fatigue, and others usually associated with flu. Sometimes there are no symptoms at all. If you're worried about it, see a doctor and have lyme titres done.

Get Rx & blood test!: A bullseye rash around a tick bite is considered proof of lyme disease.If you do not have symptoms yet, you almost certainly will over time. It takes time for a blood test to turn positive & you should not wait for this; the earlier you start treatment the better the outcome.Many lyme specialists advise Doxycycline 200 mg 2x/day for a month to be safe.Also get tested for co-infections like babesia.

Sometimes: Lyme arthritis may effect the ankle joint. If there is no swelling it is unlikely to be related to lyme.
