i had a tkr in 2008 is there immediate concern w/ pain above, below, and under knee cap and on the medial side ? i do not have an infection.

Could be  several: Things. Loosening and stress riser formation are the most likely causes. You should see your orthopedic surgeon.

i'm 3 weeks post-tkr surgery and still have pain on the inner-side area of my knee cap. i asked my doc about the pain and he said that is where the inside stitches are. my pt said it's the patella causing pain. is this normal?

Postsurgical pain: Is common and it might have more than 1 cause, meaning that they both are likely to be right. Most of the pain dissipates away or becomes tolerable if enough time is allowed for healing and strengthening. Your Ortho surgeon is your best resource going forward.

my husband had total knee replacement 14 years ago. it looks like his knee cap has now dislocated. is this serious or not?

Possibly: If he is having pain he should see his surgeon and get an x-ray.  It could be loosening of the patellar implant, fracture, or dislocation of the patella.  It may need surgery to repair.

what would cause a thickness/swelling/pain above the knee cap 5 yrs. after total knee replacement along with meds for rls are not working now?

Knee Cap: Swelling around or above the knee cap can be from the joint itself.  This can be caused by inflammation, infection, metal allergies, or loosening of the prosthetic.  If outside the knee joint at the knee cap can be bursal swelling, tear of quad tendon with hematoma. Would follow up with your surgeon that did the knee repalcement if possible.

See your surgeon: You nneed to see your knee surgeon.  You may have an infection or just inflammatory reaction.  The crunching i wouldn't worry about but you want to worry about the function of your knee.  And how much pain yo uar having...This definitely needs to be looked at!

what would cause a cyst to form under my knee cap?is there any help for nerve pain one year after total knee replacement?

Arthritis: A cyst under the patella (kneecap) will often develop as a result of articular cartilage degeneration which can occur with arthritis or other conditions that lead to damage of the cartilage.  These can be found in any joint.  The cyst is a fluid filled cavity that will often connect with the joint at the area of cartilage damage.  If the cyst has occurred after tka it is likely something else.

Possibly: Your x-rays suggest you have severe arthritis. A total knee replacement, which would correct this problem,  is one of the most successful operations orthoapedic surgeons perform. Whether or not you need surgery depends on the severity of your symptoms and your consideration of the risks and benefits of the procedure. Lump below kneecap: Depends on how long it has been there but a hard lump below the kneecap may be a condition called osgood-schlatter which is a calcification of a growth plate on the front of the knee.

Hard to say: Many joints make noise, and without accompanying pain, it is unlikely to be a cause for concern.  

Sometimes is may be an indication that there is malalignment or cartilage damage, but generally there is no relationship between the amount of painless popping and internal damage.

i have a lump about an inch from my knee cap on the inside of my knee that is painful.

Varicose Vein?: Based upon your description, the most likely cause for your symptoms is a varicose vein, which is an enlarged vein that can sometimes become thrombosed (have a blood clot in it) and can become inflamed and rarely infected.  I would discuss this with your primary physician for additional recommendations and care, but also to rule out a deep venous thrombosis.
