Pelvic pain: Pelvic pain is complex as there are several organs in the pelvis. The ovaries, fallopian tubes, uterus, cervix, bladder, appendix, intestines, colon and rectum can all be the source. Pelvic pain requires and examination to help organize the evaluation.  Low back pain has a equally long list of possible causes. You will need to schedule a visit to help narrow down the possibilities

Neural irritation: There are several causes of low back pain and thigh pain.  Problems with the hip joint can often cause back/buttock pain that radiates to the groin and thigh.  Irriation to the lumbar nerve roots in the lower back can also cause leg pain.  Thigh pain is often seen with irritation to the l2, l3, and/or occasionally L4 nerve roots.

Ticking boxes: Symptoms of radiculopathy cause one-sided back pain and sciatic nerve area symptoms.  The symptoms may be numbness, pain or weakness.  There may be reduced reflexes, especially in the Achilles tendon, if there is an S1 radiculopathy, an injury which often causes symptoms through the sciatic nerve.

Testing sometimes helps, such as an EMG test.  Physiotherapy is also a good idea - safe, and cheap

Herpes type I ?: Herpes types I and II are usually self limited ulcers which are painful but do not generally cause "horrible back pain".  Shingles, however, which comes from Herpes Simplex (latent "chicken pox") can cause very painful skin lesions and lower back pain. See your physician for testing to be certain.

Many: Consider the right lower abdomen and back and these include your appendix and yes, it can be to the back (posterior), the ureters, the kidney, and the stomach and in women the adnexa or ovary. There could also be referred pain. The chills is concerning for infection or even a stone. Go have a good evaluation and be treated appropriately.

: Focal muscular spasms can indeed cause secondary asymmetries of the lumbar spine, but these are triggered by underlying pathology, such as discogenic protrusions or herniations, facet impingement, trauma to muscle, even secondary sacroiliac dysfunction.  A physiatrist can help provide diagnostic studies and rehabilitation interventions.
