Have you been seen?: First if you have never been seen for this ,it is time to go see the doctor.  It is better to apply cold to the area until you understand what is wrong in the back.  Muscle vs nerve is addressed with the physical exam.  Treatment is based on the working diagnosis after physical exam and maybe xrays.  Could your pain stem from kidney, uterus, bladder?

i took 4 advil (ibuprofen) for my lower back pain. it isn't helping, is it ok to take a vicodin?

advice is no: Generally speaking i would avoid narcotic pain medications for low back pain: things such as vicodin, percocet, percodan, (oxycodone and aspirin) oxycontin.  I would stick to nsaids such as motrin/naprosyn.  The narcotics may make you feel better in the short term but can lead to many problems.

Exercise: Most low back pain is mechanical in nature. Stretching and strengthening exercises for the lower back go a long way to improve low back pain. See a physiotherapist who can teach you the specific exercises.

Back pain: Back pain can be caused by a number of factors including muscle strain, ligament injury, nerve irritation, vertebral injury or inflammation or internal organ issues. If the pain came without any clear ttrigger it is more worrisome. Could be something like a kidney stone. If medication not helping some therapy might.  See your doctor for this to see if further imaging studies may be indicated.

Work up needed: While the majority of times causes for back pain are benign and pain becomes chronic, whenever the pain is no longer responding to conservative treatments further work up is needed, as even benign causes may progress enough to cause significant pain and functional limitations. Starting with your doc is recommended.

Yes it can: With GERD, repeated use of ibuprofen can cause erosions and lead to ulcers in your esophagus or stomach. At least you should use a GI protectant if you need this drug, and regular evals for anemia, kidney and liver function. Remember most GI bleeds are painless.

i have been on accutane for 6 weeks on a dose of 1.5mg/kg and started getting lower back pain 2 weeks ago. should i be worried and is it permanent?

Usually no.: Accutane is a very powerful medication with significant side effects including joint pain. Once you stop the accutane, the joint pains, in your case back pain, should slowly resolve back to your baseline. You should report any side effects, however, to your dermatologist who is managing your accutane.
