Silymarin, the active extract of milk thistle, has been shown in double, blind placebo controlled studies in us & europe to be an effective antioxidant and helps treat or alleviate condition esp. Smoking should not: have any effect of which I am aware.  The most important question is the source of the elevated ALT, which statistically speaking is likely hepatitis C or alcohol, or the combination, although there are hundreds of other causes.  Silymarin, while widely promoted, lacks evidence of effectiveness in several randomized controlled trials, but couldn't hurt. If its hep C, GET TREATMENT. Alcohol? - STOP

Not all once daily: Atorvastatin should be taken once daily.
Orlistat 120 mg is usually dosed three times a day with each main meal containing fat. The dose may be taken during the meal or within 1 hour of completing the meal. Check with your physician for details regarding your recommendations.

 UsuallyAlprazolam does not affect the Liver function.

Prescriber dependent: Dosage and duration of silymarin depends on the reason for your hepatotoxicity. The studies I have read dosage has ranged from 20 mg/kg to 33 mg/kg body weight, duration ranging from 3 to 8 weeks. Studies are mixed in showing efficacy from robust response to no response, as shown in a large study published in jama july edition on response to slymarin in patients suffering hep c. Talk to your doc.

Liver and thyroid: Fatty liver is common with obesity/metabolic syndrome.  Best treatment:  weight loss, any way you can get it.  I am not a fan of orlistat.  Have you read about the side effects?  Most people don't take more than a few doses. I doubt milk thistle (silymarin) will help, but it is harmless.  If you are hypothyroid, you need to be on levothyroxine replacement.

Elevated LFT's: If you continued your fast food diet but expected milk thistle to correct the effects on your liver, you now have evidence that this didn't work.  There could also be problems with the preparation you used, as some have been associated with elevations in alt. http://tinyurl.com/krz5efy  drinking alcohol heavily or large quantities of liver-affecting medications like tylenol (acetaminophen) could cause this too.
