I see listed: Conditions as ocd, anxiety ; adhd but i don't know if you are receiving any medications or therapy for this. Start with a medical eval. It may uncover underlying medical problems that need to be addressed.  Seeing a therapist cognitive behavioral therapy can be quite effective. Psych meds may be indicated. Aim for daily exercise, 8 hours of sleep per night ; good nutrition w 5 servings of veggies.

Gynecologist: We do not have access to doctors in London but PCOS and endometriosis are both common problems. Any Obgyn in London should be familiar with the treatment options for both of these problems. At 42 many patients might consider a hysterectomy for stage 4 endometriosis. If your Obgyn does not do surgery they can refer you to one who does.

Most likely cause of: This pattern of symptoms is inadequate fluids or excessive caffeine or alcohol intake. Should be drinking 80-100 oz of caffeine free/alcohol free fluid daily. If you do this , will take 7-10 days to get some consistent results.

Chronic cough: The three commonest causes for a chronic cough are asthma, nasal/sinus conditions and gastroesophageal reflux. Thick secretions might be causing the cough to make you feel like you'll vomit. Wheezing sounds like you are in the asthma area. Even without insurance, you can get care. Try calling your county's medical society for guidance to a free clinic or federally-funded community clinic.

Oral sex: There is very little risk to performing oral  sex on a female. One  can get genital herpes this way if the female is infected with this. Otherwise I have no concerns.
