Had a cath?: The echo says there might be a structural abnormality of the aortic valve and a leak of the mitral valve with resultant dilation of left atrium. If you had the cardiac cath you list any time recently, more information should be available from that. A TEE is a transesophageal echo which gives us better pictures than a transthoracic echo. You should discuss this echo with your Doc

: A dilated left atrium usually means there is pressure built up there. Why is there pressure build up there ? valve issues? hypertension?  Its common in an obese patient who has hypertension. It is known to be connected to atrial fibrillation

LAE: Left atrial enlargement is actually common in aortic valvular  stenosis but not as severe as in mitral valvular regurgitation or longstanding hypertension. This is why I feel that atrial fibrillation is less common with simple aortic stenosis compared to those other causes of LAE.  Aortic valve replacement should allow the LAE to regress.

Reason?: Depends on the reason? Do you have hypertension, mitral stenosis, mitral regurgitation, left ventricular hypertrophy, atrial fibrillation? These are all treatable. In all cases, you're at increased risk of developing atrial fibrillation and congestive heart failure. That doesn't mean you'll ever get those-your risk is simply increased.

Prognosis: a personal visit with a doctor would be the best way of learning about  the prognosis; However an ejection fraction of 40% is somewhat low, a cardiac defibrillator is usually recommended when the ejection fraction drop at 35% or below to prevent sudden cardiac death

Depends: Isolated dilatation of the left atrium is very unusual and is often associated with something else like a mitral valve problem.
