Lactation problems: Refers to challenges with breastfeeding. Can include problems with latch and milk transfer to infant.  Also can include inadequate supply for a variety of reasons. Also can include breast infection- mastitis as well as yeast infection on nipple.

Lactation specialist: Lactation specialists are usually nurses that have experience in helping mother with their nursing issues. You can get referral from the nursery nurses, or look them up in the internet for one near you. There is a volunteer organization called "la leche league" that also helps nursing mothers all over the world.

Not sure: There are several causes of "lactation problems" from the lack of prolactin hormone stimulating the milk ducts to inadequate sucking by the child. High anxiety mother's are generally less successful at breast feeding than mother's who are less anxious.

Pregnant or not?: The question is not very clear. Is the lactation problem post-partum and involves breast feeding? Initially there is colostrum and the full milk supply does not come in for several days warm compresses and a breast pump could help.If you are not pregnant and secreting from your nipples, a serum prolactin level and mammogram might be in order.

Reglan (metoclopramide): Reglan (metoclopramide) is sometimes used off label for this.  In my experience it improves lactation about 50% of the time.

why isnt oxytocin available for those with lactation problem?

Accepted Answer

It is dangerous: Oxytocin is used to augment labor, and is also the hormone that is involved in let-down in a lactating woman. It does not really increase milk supply as much as it allows the milk to release. It is only used in labor under very careful monitoring because it can drop someone's blood pressure. Fenugreek, metaclopromide, and pumping are better options to increase supply.

will breast biopsy make your breast damaged and possibly have a problem in lactation in the future?

Not likely: Usually a breast biopsy is by stereotactic means. This is the insertion of a needle into the breast. The amount of damage done to the breast is relatively small and should not significantly impact on the overall function of the breast in producing milk. If the biopsy is via surgery the damage may overall be more but again should not impact on the overall function.

Depends: Most large hospitals have them and you can call around.  Also, doula's are trained in this.

Fluids, Fenugreek: The best idea for healthy breastfeeding is to stay well hydrated...Lots of fluids! even in the best of times, breast milk volumes can decrease, and there a lots of home remedies. One of the herbs that has been studied recently is fenugreek --- and when purchasing fenugreek, be sure to shop at a large, reliable health food store or pharmacy that has lots of turn-over of its products for freshness.

Can help yourself: Lactation or breastfeeding headaches can be caused most commonly by weaning too quickly, exhaustion, dehydration, poor diet, stress, thyroid changes and hormonal changes, estrogen and oxytocin (control of milk release). Try to relax, eat a well balanced diet, and drink plenty of fluids.
