Early Pregnancy: At 233 hcg level, it is impossible to see an intrauterine pregnancy. So check your hcg again in 48 hrs and if it doubles, it is a good sign. If not you may have an ectopic Pregnacy. Take Tylenol (acetaminophen) for the pain if the level is 5/10?or more. I recommend you go see an Obgyn soon as well

Stop bleeding: Anemia is not a normal clinical situation.  It means for the most part that there is an active source of bleeding which must be stopped.  Vaginal bleeding needs to r.o. endometrial thickening or Ca.  Rectal bleeding is from tumor of distal bowel when spotting bright red or stomach when black.  Work up requires defining site of bleeding and then characterizing lesion with bx to choose treatmen.

Your hormones are in: .. flux for some reason, so hang on for the ride.  Irregular periods and spotting could mean you may have some hormonal imbalance that could be affected by many different factors, not the least of which could be your implanon.  They are supposed to last for 3 years continuously -- but sometimes that doesn't happen.  Talk to your GYN about your irregularity and check a preg test while you are at it

Get An Exam!: There are many possible side effects with Mirena (levonorgestrel) coil including bleeding and spotting between periods, heavier bleeding during the first few weeks after device insertion,  and missed periods (amenorrhea). You should have an exam by a gynecologist as soon as possible.

Check 4 pregnancy: If you miss more then one pill per cycle pack you are at risk of pregnancy, so start check for that.  The spotting could be implantation bleeding.  Irregular pill use will also cause irregular bleeding and sometimes cramping.

YES: Since different oral contraceptives contain differing amounts of various hormones, you body will try to readjust to the new hormone levels. This may result in spotting, irregular bleeding or no period at all while your body adjusts. this can take up to 6 months if the hormone levels are very different between the old and new contraceptive. If the spotting persists beyond 6 months, talk to your DR.

About those pills: Plan B (and other "morning after" pills) can alter the menstrual bleeding for the cycle in which Plan B was taken, and sometimes in the next cycle too. One should be sure to check a home pregnancy test about 3 weeks after sex, because emergency contraceptive pills work most of the time, but not all the time.
