Acl/pcl healing: If an acl and PCL tear these ligaments do not typically heal; this is due in part to the synovial environment in which these ligaments reside as well as the demand that is exacted on these ligaments. Whether one is symptomatic with these injuries is in large part measured by the degree of activity in which a patient participates in and the number of hours one is actively engaged.

Arthrofibrosis: You have what's called arthrofibrosis caused by your prolonged immobilization. The next step for you is physical therapy aimed at restoring your range of motion. If this is not effective you may need a manipulation under anesthesia or an arthroscopy to break up the scar tissue. Let's hope you get it accomplished through physical therapy. Good luck!

Variable: A PCL partially heals in a slightly looser position. Some patients have minimal difficulty with a PCL tear while others have instability sensations and weak push-off strength. Still others have rotational instability. Because of this, the PCL discussion needs to be personalized to your specific situation. This should really be addressed with a PCL sports medicine specialist.

See below: Strengthen the surrounding muscles of the knee as well as you can.  This will help keep the injured ligaments in place due to the increased muscle tone surround the knee.  Obviously, the older you are , the harder and longer this may take.  See your orthopedist and physical therapists for more help.

ACL-deficient knee: The short answer is yes!  an acl-deficient knee is inherently unstable and thus will allow "wobble" (or hypermobility) - similar to loose lug-nuts on your car's wheel.  If you completely remove 1 of the lug-nuts you'll increase the stress of all the others that are trying to stabilize the structure.

PCL tear: With isolated PCL tear, current recommendations are physical therapy with quadriceps and hamstring strengthening. If he has a concomitant acl tear, he may need a reconstruction of both. If there is pre-existing arthritis with malalignment of his knee, he may require a total knee replacement. It can be a complex algorithm depending on the alignment of his knee and what his x-rays show.

See orthopedist: An experienced orthopedic surgeon can guide appropriate therapeutic directions.  You might start with injections, such as cortisone, and/ or hyaluronic acid.  Surgery could be arthroscopic, but perhaps open.  Physical therapy is a terrific tool for rehabilitation.  Discuss these steps with the specialist.
