Probably more a: soft tissue problem like a tendonitis, common in CP.  Talk to your doctor about this.

Many things: If you have no pain walking, unless you walk very slowly, poor circulation seems unlikely.  Most of my patients experience more pain when they go to bed, what ever the condition happens to be.  I think that is because we are distracted during the day and we aggravate many conditions with daily activities, so it hurts more when we go to bed.  You may have arthritis, a pinched nerve, a torn meniscus

Be seen by MD: Please be seen by your MD.  Have you had any trauma other than your fracture 25 years ago .  You might try placing a pillow under your knee to keep your leg at an angle overnite.  If pain is severe, be seen today and do not take a chance.

Arthritis: You may have arthritis, or a tear of the meniscus which has allowed a cyst to form behind the knee.  Would recommend an evaluation.

A few thoughts: You have to consider what is behind the knee. There are blood vessels, tendons, muscles, knee joint. You have to consider each of these. When you are evaluated these will be looked at to try to clarify your symptoms.

Could be: anything.  But most concerning is venous thrombophlebitis which can lead to pulmonary embolus.  Have your doctor take a look at it.

Sciatica: True sciatica is leg pain that shoots from back, down the leg past the knee. This mimics the path of the sciatic nerve, thus it's name. Mri of the spine or pelvis would serve to identify in most cases if exam and history is not clear. Herniated disc at higher level like l2-3 or l3-4, could cause pain to knee. Lower disc problems calf and foot. Nonsurgical for most disc problems. Nsaid's, pt, shots.

Meniscal tear: There are many potential causes.  One of the more common is a degenerative meniscal tear.  Don't hesitate to be seen and thoroughly evaluated.

Less likely: A meniscal tear is usually most painful with weight bearing. See an orthopedist and have the knee evaluated. Then you will have your answer., .
