knee pain after tramping for hours then going downhill, uphill is fine.  outside of knee horizontal line in middle. as i lift & lower my leg to step down, not when my weight goes on it. how to fix?

Could be: A few things depending upon a history of prior injuries or surgeries, but the discomfort particularly going downhill makes me wonder about the acl (anterior cruciate ligament) and the patellofemoral joint,  makes me suggest you see your doctor for an exam.

Orthopedic specialis: If the discomfort is definitely arising in the knee and only occurs while in bed, there is a positional component to it. It may occur when leg muscles are completely relaxed and, at this point a cartilage, tendon or ligamental structure may become impinged. An orthopedic surgeon is best qualified to evaluate and advise you. A knee X-Ray or mri may be recommended in order to make a diagnosis.

Need evaluation: There are many different possibilities as to the source of the pain.  It could be due to a pinched nerve from the lumbar spine, sacroiliac joint dysfunction, hip disorder , muscle strain, or many other causes. Discuss first with your primary care physician to initiate the work up (physical exam/xrays ) and if the pain persists/progresses a referral to a spine specialist may be necessary.

: You need to have this evaluated ASAP. If you go to an ER, they can evaluate it via x-rays and CT scan or MRI as well as examine it to find out what is wrong. Orthopedics can also be called in to see you, depending on what the issue is or you can see them in the office with the test results so they can diagnose and treat the issue. There are many possibilities so an evaluation is needed. Good luck!

Painful kneecap: You have described some on the classic symptoms of patella-femoral pain or chondromalacia patella. It is an inflammation under the kneecap. The pressure between the kneecap and femur is greatest when going downstairs or hills. See an orthopedic surgeon for evaluation. Often can be treated with just physical therapy alone, but may take unto 2-3 months to effect a cure.

Knee pain: Knee pain can be from multiple reasons.  Usually it is from an injury.  If your knee hurts that bad and you can not stand on it I would get it checked.

Hard to say w/o exam: But first things to come to mind are hip adductor strain/ groin strain and arthritic or other changes in the hip itself (commonly refer pain to the knee). Try some over te counter nsaids for a shirt time, but if it doesn't help, see your doc.
