Tough Question: Boy thats a tough question. Usually things that are bright on ultrasound are calculi (stones), air, or fat. If it were air the pt would likely be very sick. Fat is almost always benign, and stones can cause symptoms ranging from mild pain to sepsis.

kidney ultrasound measurements were 89mm x 42mm x34mm on left and 89mm x 33mm x 45mm on right.  i'm 34, female. should i be worried?

KIDNEY SIZE: Kidney size can be variable, the kidney function is more important. You need an exam and labs. Especially bun/Cr ratio. 24 urine for protein and creatinine clearance. Follow up with your doctor and potentially a renal consult .

that depends: If they are not causing symptoms, probably nothing should be done at this time. If they are very large or causing obstruction, a urologist should be consulted for possible extraction or lithotripsy. Also- you should consider seeing a urologist anyway to have a complete metabolic work-up to see why you form stones. There may be treatments available to reduce the formation of future stones.

does loss of cm differentiation on kidney ultrasound only indicate a chronic problem or can it be acute too? or is it non-specific?

: It reflects the scarring or fibrotic change of the renal parenchyma (the functioning part of the kidney) which takes months to years to develop, i.e. chronic.

: If you are asymptomatic then this is an operator driven inaccuracy. If you are worried pl see a Urologist.

Flowing blood: It seems you saw the screen during an ultrasound. One of the functions uses doppler analysis to determine blood  flow. Flow is indicated by color on the screen.
Yellow is usually used for power doppler which is more sensitive to flow but not direction specific.

Doppler: A regular ultrasound shows shades of black and white to identify kidney sizes and structures. 
A doppler senses flow to (red) and from (blue) the probe.  Doppler ultrasound is used to rule out renal artery stenosis and venous obstruction in the kidneys.

Stone? Pregnant?: If you had the ultrasound for acute pain you might have, or passed, a kidney stone. If you are pregnant this mild distention can occur as a normal part pregnancy as the uterus enlarges to accommodate the baby. Best to followup with a urologist to be sure.

: CM diferentiation = CorticoMedullary differentiation. By renal US, the cortical and medullary sections (outer and inner) have different echogenic charaters and that difference is showed as corticomedullary differentiation. If the cortical section (renal parenchyma) is damaged and scarring (chronic) the echo character changes so there is no more echo differentiation between the two sections.
