Itchy nipples: You may have eczema, which can affect soft sensitive skin. If you have any other part of body with dry skin patches, it would be a likely cause. You can also think about superficial fungal ingal infection called ring worm. An exam would be helpful if OTC meds dont help resolve the condition.

May be herpes: Herpes can initially cause itching. That's often followed by a blister that ulcerates. May cause pain, especially on 
urinating. The only way to tell if HSV is to do a swab of some fluid from the sore. Since you were able to see the "pimple" I'm not sure why you are asking if it easily visible. The doctor can use a magnifying glass to view it if needed.  Best wishes. Always use a condom.

See derm: See dermatologist since there are many causes for itching. In the meanwhile, be sure to apply a moisturizer immediately after bathing to see if this helps since dry skin is by far the most common cause for itching.  On the other hand, the rash needs to be examined even though it may come from repeated scratching.

Tucks (witch hazel) medicated pads: You may have internal hemorrhoids. I would use tucks (witch hazel) medicated pads for hemorrhoids for the itching and preparation h ointment. CONSIDER a stool softer like colace. Don't sit for long periods of time. Get a donut shaped pillow to sit on.

Scabies treatment: The time of day itching occurs is not affected by the treatment of scabies. Itching is caused by an allergic reaction to the mite and it's feces. Generally, itching is increased with warmth and therefore more pronounced in bed. An increase in symptoms after treatment might be due to an allergic reaction to the medication and should be discussed with your doctor.

Possibly: One of the adverse reactions that some people get from Lipitor (atorvastatin) is urticaria, a fancy name for the hives.  Typically, urticaria is associated with raised reddened lesions that itch.

Yes: The elevated bilirubin is what causes the itching.  The bilirubin is deposited in the skin, that is what makes you yellow, and it itches.  There is nothing you can do except wait until your liver gets better and the bilirubin drops down to normal.  

In the meantime, taking an antihistamine might help, but you have to watch for sedation.  Talk to your doctor about that.
