: I wonder why you are seeing a cardiologist at your age. In general, total cholesterol of 240 at your age is not much a concern, especially you have high HDL. Your cardiologist is correct and you do not need to lower total cholesterol to below 200 if you are otherwise healthy.

Potentially: If you have a very high number of small particles in your LDL which can only be determined by fragmentation of the LDL by nuclear magnetic resonance, nmr, or spectrometry through regular blood work sent to your lab or to the berkley heart lab in ca.  Specially meaningful if there is a strong family history of coronary artery disease.

Probably not but...: Contrary to popular belief, cholesterol is not a major cause of heart disease; the real cause is inflammation.
However, it is reasonable to look at the subtypes of LDL as if it is mostly the small dense type it is of some concern. You should test other risk factors like CRP, homocysteine and Lp (a).

See below.: Assuming yno early family history of heart disease or personal history of diabetes, stroke, or heart attack, then: your total cholesterol is a bit high - however, this is due to abnormal numbers for the rest.  Triglycerides (fats) are high - reduce your fat intake.  Hdl is about right but could increase a bit more to 45.  Ldl is on target.  If you have the above risk factors, then ldl<100, tg<130.
