In diabetics use of metaformin  may interfere  with Amino Acid homocystein metabolism and  lowers B12 level and trigger vascular thrombosis, excessive levels may thicken the blood  ( polycythemia)  and may cause clotting.

Vitamin B12 level: Can be high for several reasons: 1) you are taking vitamin B12 as a supplement or eating a diet high in b12.  2) your body cannot metabolize B12 so it stays in your blood stream but does not get into your cells where it needs to go for it to work.  There are tests to determine if the latter is the case.  Treatment would be geared towards getting the B12 to be utilized by the body appropriately.

Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex.

No: That is at the upper end of normal or a little out of range on high end depending on which lab you are using. Levels that high are usually associated with supplementation. Depending on how much, how frequent  and by which method ( oral, sublingual, injection) you can back off on dosing frequency if you like. If you feel better keeping it at that level then no harm done.
