Depends on diagnosis: Amitriptyline (elavil (amitriptyline) for brand name) is an anti-neuropathic medicine.  It is an old generation anti-depressant.  Neurontin (gabapentin) is also an anti-neuropathic medicine.  Neurontin is also an anti-seizure type medicine.  Patients with chronic nerve pain have different reactions to anti-neuropathics.  Neurontin seems to have more specificity toward nerve pain.

Neurontin/hand pain : There are many reasons for hand pain. If there is suspected nerve problems causing the pain, Neurontin (gabapentin) may help. Swelling will not be helped and Neurontin (gabapentin) can actually cause swelling in the hands, legs and feet in some people.

Practicing good sleep hygiene, having a routine  in place, will help with sleep. Also, you might want to discuss your insomnia issues with your providers to see if sleep study is warranted. Treat the cause: Most important is treating the source, which often remains elusive or damage irreversible, necessitating need for symptom treatment. Many types of nerve pains exist & making the distinction guides better treatment. Combo therapy is often needed augmented w/ less traditional modalities such as topicals & acupuncture. Interventional pain procedures is a candidate. Opiates work for refractory cases.

Neurontin (gabapentin): Neurontin (gabapentin) is a safe drug and very helpful for neuropathic pain. Neurontin can cause drowsiness and dizziness when first started so beginning the medication at night to minimize side effects. The dosage can be increased every 3-5 days until the nerve pain is well controlled and as the side effects resolve. About 8% will develop swelling and weight gain after a few weeks of therapy.
