Not recommended: The combination of the these two medications together can potentially increase levels of serotonin which can lead to serotonin syndrome which can be life threatening.

Not recommended: Tramadol and alcohol are depressant of the central nervous system. Mixing tramadol with alcohol could lead to drowsiness to severe depression of brain activity, coma, and ultimately death. Tramadol has a prolonged half-life about 7 hours. It is still in your system for at least 10 hours. Is not recommended.

Sedating medications: Elavil (amitriptylene), tramadol, and Flexeril (cyclobenzaprine) are all sedating and if taken together in someone who's not used to them could indeed make you feel ill.  The Mavik (trandolapril) is a blood pressure medication which is not sedating and is not culpable here.  Both tramadol and Flexeril are as-needed meds, so it is safe to reduce your own dose of these by dividing the tablets and taking 1/2.

Tramadol: Low dose tramadol may be used initially while the Butrans efficacy is being evaluated. It can also be used as a medication for any exacerbated pain that you have after the Butrans patch dose is optimized. If you are fine with Butrans alone then no need for Tramadol.
