IUD ?'s: If you have had a relatively uneventful time since IUD insertion, the IUD may now be in the cervical canal b/c of uterine contractions.  Also, IUD's do NOT PROTECT against tubal (ectopic) pregnancies, so if pregnancy is possible, obtain a serum HCG test to be certain, and if positive, see your gyn physician immediately.

Vag. lubricants: Most women can use vaginal lubricants to reduce vaginal irritation.  E.G. Lubricants such as k_y jelly are applied to the vaginal areas to decrease friction from sexual activity  in sensitive women.

Vag. lubricant : Those persons who are allergic or hypersensitive to the ingredients of the vaginal lubricant.

Follow directions: You are seeing a physical therapist so it will be very important to understand the exact exercises he or she wants you to do. The dilators may be an important part of you therapy but you want to make sure you are using them as directed by your therapist.

If : If you have faithfully been taking your birthcontrol pill as you described, than i believe you have effective contraception.  Of course no birth control is 100% effective.  It would be reasonable to have him finish intravaginally.  I do not believe Plan B would add any further protection for you.  
Ps: of course safe sex practices should be considered.  A frank discussion about previous sex history needs to be discussed with him too. Be safe.

Yes: But don't go from anus to Vagina due to cross contamination of the vagina/urethra with colonic bacteria. Like you have been told to wipe from front to back

Third degree: Tears generally heal well after delivery when repaired.  There is a great deal of blood flow to the area, speeding recovery.  I wouldn't do anything extra unless advised to do so.  If there is a problem, then physical therapy.  Good luck.

Unclear: Vaginal estrogen is sometimes prescribed to post menopausal women for atrophy. The use of vaginal Estrogen is unrelated to one’s need to undergo an annual preventative cancer screening exam such as a pap smear.
