It can be.: Many people will have some degree of misalignment between their two eyes when fatigued.  This is called a phoria.  It is normal.  Patients will experience double vision temporarily, but should easily be able to eliminate the double vision by "refocusing".  Double vision that does not go away or happens frequently is abnormal and should be evaluated by an eye doctor.

Accepted Answer

Divergence palsy: When you look at a distance, the in-turning muscles relax, the out-turning muscles tighten. In your case, the out-turning muscles are week- could be medical side-effect, stroke effect, decompensated eye crossing, other neurologic disorder.
If the amount of prisms becomes excessive, have eye muscle surgery.
Contac a "pediatric" ophthalmologist who does a lot of muscle surgery.

: Consult with a neurologist and see if there is an issue with intracranial problems.  Medication side effects could be playing an issue.

suddenly can't see out of eye the other day i was watching tv and i started having double vision. then the next day i woke up and my left eye was blurry and i couldn't see out of it at all. i've been getting migraines my entire life and a few hours after

The : The symptoms are not typical for a migraine alone. You noted two distinct and dramatic  symptoms (double vision, blur), separated in time, and i think you indicate a headache as well. More history will help to zero in on the problem, and if you were my patient i'd have you come in promptly (i.e. That day) to figure out what's going on. While migraine (which can change in presentation over time), loss of control of a pre-existing eye muscle imbalance, and/or surface irritation would be the most benign cause, other, time sensitive and potentially dangerous situations should be investigated immediately. You might have a vascular cause needing immediate attention (e.g. Transient ischemic attack, temporal arteritis or other inflammations) or other issues. My advice: see your eye md asap.

Accepted Answer

Usually not.: Unless there is a big difference in the prescription between the two eyes, you should not experience double vision when wearing your glasses. There may be a problem with the way that the glasses were made or the prescription itself. It could also be something unrelated to the glasses. Your complaint warrants further investigation by the individual who prescribed your glasses.

Depends: Right after surgery or has it been awhile?  If eye muscle surgery was supposed to correct it but did not, then could be under/over-corrected, or could have alignment issues in more than 1 direction that was only partially treated, or other causes.

when i lay down and watch tv or use my ipod for a while, i start to see the words double like if i had diplopia. vertically, i see 2 of the same words?

Double vision: Some people confuse double vision and blurred vision.  Double vision is seeing two distinct, separate images of the same thing.  Most double vision is due to misalignment of the eyes.  Double vision with fatigue is common in people who have a tendency for misalignment - called a phoria.  If it occurs frequent, a correction in glasses called prism can done to reduce or eliminate it.

Check your vision: A shadow doesn't necessarily mean "double vision", it could be a manifestation of out of focus vision.  If the trouble is only at near, then it may be presbyopia.  Have a thorough eye exam to determine the cause.

i was tired reading my phone. suddenly, i noticed the text had a small "shadow" of the text underneath it down and to the right. no double vision looking at objects in room. just text on contrasted backgrounds. went away in ~5 min. no other symptoms.

Not to worry: You are probably spending too much time reading your phone. Give yourself short breaks in between and look at things far away.
Wish you good health! - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex.
