Urethra still patent: Unfortunately closing off urethra or bladder neck is both difficult & has a high failure rate. Virtually impossible to do from below & best performed suprapubically by open surgery from above. Bladder neck has to be inverted & closed in 2 layers. Possibly your urologist could inject coaptite around bladder neck cystoscopically via the mitrofanoff channel using a pediatric cystoscope.

Many possibilities..: Can cause incontinence in the elderly; grouped into overflow, urge & stress incontinence. Inability to void regularly, low capacity, poor contraction, and poor sensation can cause a full bladder to overflow. Infection, overactive bladder &  stroke can cause urge-related leakage. A poor urinary sphincter will leak. A full evaluation by a urologist can help find the cause and manage the situation.

Urodynamics: We do a urodynamic test on all women reporting urinary incontinence that is bothersome. Frequently there is anatomic problems from childbearing that are mixed with bladder muscle spasm as the cause. The anatomy can be fixed, yet muscle problems may remain. They respond to physical therapy, neocontrol treatment. Medications can help but only treat symptoms rather than fixing the problem.

The : The only way to be sure of the status of your urine is to have a formal urinalysis(ua) done by your doctor. Multiple things are screened for in a ua including protein, glucose, nitrites, white blood cells etc. Your doctor will be able to interpret the results and decide on additional testing. You may need an urologist evaluation in order to determine if there are any abnormalities of your urinary tract, which would explain the change in your stream.

See below: Clotrimazole (antifungal) most commonly used to treat a yeast (Candida) infection involving the head (glans) of the penis and/or foreskin. Most common in uncircumcised males and diabetics. Application to the meatus a little unusual. Regardless of reason, pts usually recommended to apply once or twice daily "after" cleansing area with soap and water and drying before application.

: Female incontinence is common especially if you have had multiple vaginal pregnancy with delivery. Usually it will spontaneously end but you might feel safer if you had a pad in place for when you strain, cough or sneeze.  There is a national organization for this which you can look up on the internet if the condition becomes persistent.

Probable infection: It sounds like the most likely possibility is a urinary tract infection. You should see your doctor for a checkup and appropriate tests.

: Not sure if you actually leak or have the feeling. When you think you have finished voiding- remain on the toilet and try to empty some more. (double voiding) Avoid constipation. Good luck

See below: She needs to be seen again.  If she is having pain with urination, fevers, increased frequency of urination and urgency or urination, get seen now to rule out a urinary tract infection.  But she still needs to see a neurosurgeon to help determine is the sacral dimple is associated with the spinal cord and the problems she is experiencing.
