Diabetic specialist: Diabetes is a very difficult illness to treat because it requires u to take care of it every day. I would recommend seeing a diabetic educator and/or reconsult your doc for further evaluation, or even seeing an endocrinologist. Continuous glucose monitoring is available and Insulin pump can help reduce #of shots etc. There are many options now--where there is a will, there is a way. You can do it!

Hypoglycemia: If your blood sugar is low before you eat then yes, eat your meal and check blood sugar about an hour later.

Blood sugar: Rapid acting insulin lasts from 4-6 hours and reaches peak effect in 2-3 hours at which time your blood sugar would be at its lowest as a general rule. However if you had taken a long-lasting insulin earlier in the day, it would also be exerting an effect hours later. Your physician who knows your case best is also best qualified to answer your question.

You can, but do not: need to. Taking carbohydrates in moderation is okay.
Wish you good health! - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex.

what could be causing my blood sugars to drop even after eating. im not on insulin only metformin?

If not of diabetes: Treatment, in my 35 years of practice, I have never seen a case of reactive hypoglycemia.  The disorder doesn't exist, if you're overweight the symptoms are a natural response to your glucose level returning to normal. They do not drop below normal or to dangerous levels. Finger sticks are unreliable don't fall for any fad diets or treatments. Lose wt if heavy.

5 hr gtt: You need a 5 hour glucose tolerance test. Your notation that "glucose is normal" and "post-prandial Insulin is 66" may be mis-timing the changes that occur during your symptoms. These changes are quite dynamic, changing over minutes.

LOW sugar!: 18 mmol sugar reading is LOW, especially after a meal. If his sugar is in this range often he needs LESS insulin &amp; you should consult his Dr right away because low sugar can lead to coma if excessively low . Some diabetics will have lower insulin requirements if they have kidney disease, weight loss, or other issues .  37 degrees C is a normal body temperature.

What's your question: Why was your serum Insulin checked in first place? What's the reference range given for insulin? How did your ordering physician interpret test for you? Why was blood drawn after a meal b/c you need to be fasting in order to calculate your homa-ir? As you can see, too many questions need to be addressed before we even get to your question, whatever it is!
