Make appointment: Ingrown toenails and other minor toe wounds and infections are a leading cause of limb amputations. Sounds crazy and dramatic, but the underlying vascular disease is the reason. In order to heal any wound you need extra blood flow. If you have vascular disease, you won't get that extra flow and your wound won't heal. Go see your doctor, ask for an artery ultrasound and referral to vascular.

You're : You're describing something called a pyogenic granuloma.  It's not all that uncommonly associated with an infected ingrown toenail.  When an ingrown toenail is removed (more on that later), any granulation tissue present is usually also removed while your toe is numb, and whatever material is used to cauterize the nail root is also used on the pyogenic granuloma to cauterize that as well.  Between that and a course of antibiotics, the pyogenic granuloma disappears.

When i say "removed, " i mean the little procedure to remove that piece of nail that's inside your skin followed by some kind of cauterization.  Short of this, merely cutting away a corner, or anything that can be done without the need for anesthesia, is doomed to fail.  At best, you'll get some temporary relief, but it almost always returns.

More and more doctors are offering some kind of discounted fee for the ever-growing population with no health insurance.  We charge $45 per visit, which includes any procedure, and since an ingrown toenail procedure is, on average, a 2-visit event, uninsured patients end up paying $90 to get their ingrown toenail fixed.  I'm sure if you make a bunch of calls, you'll find someone willing to reduce their fee to get this thing done right, once and for all.  Soaks and advil (ibuprofen) aren't going to cut it.  You really must go to a podiatrist and get the thing fixed.

Gangrene: would be rare for a young and healthy patient to get with simply soaking in vinegar for a short time... however, if a patient has limited blood supply and/or has neuropathy in the feet, then the chance of gangrene is exponential.  A picture or exam would give you the best diagnosis and treatment plan you may need!

Have it checked: What you describe is a condition known as onychocryptosis or ingrown toenail. The symptoms can vary from very mild to severe depending on how long it has been present. It occurs when part of the nail penetrates the skin, which can often result in an infection.   The treatment includes antibiotics, and removal of the offending part of the nail.  See a dermatologist or podiatrist  for evaluation.

Ingrown toenail: Use warm compresses and ice for 20 mins a piece.  20 mins warm.  20 mins ice. You will likely need An antibiotic and someone to fix the ingrown toenail.  A podiatrist would be a good choice or ask you family doctor if they can help you .  You maybe able to get an antibiotic by calling your doctor on call this weekend.  Good luck and feel better!

Sounds like it.: Get to a Podiatrist ASAP.  Your partner most likely has an infected ingrown toenail which should be treated promptly.

Gout, pseudogout,: infection such as GC are major causes of non-traumatic mono-articular arthritis.  You need a medical evaluation if pain is this severe.

Podiatry.: I would encourage you to get up the courage to see your podiatrist.  If there are fragments of nail growing into your skin - the infection will continue. The procedure for an ingrown toe nail is really not that bad.  What if you develop cellulitis or the infection travels in your body?
