i recently noticed a small itchy whitish-red patch (a single patch) on the inner foreskin of my penis. i have no other symptoms like inflammation of the glans/foreskin, etc.

what can the patch be and how should i treat it?

Foreskin: This could be an early manifestation of a condition called lichen sclerosus which is usually itchy. If this is what it is, it can progress and eventually result in phimosis from scarring unless it's treated. I recommend consulting a dermatologist or urologist for proper diagnosis and treatment.

Remove or preserve?: Physically stretching a tight foreskin over the penile glans can only lead to skin splitting, then scarring and ending up with a worse problem. Steroid cream, such as Betamethasone , when applied twice daily for many weeks to the tight area may loosen things up. 50% will tighten up again after applications cease. Alternative is a minor dorsal slit through the tight area, this preserves foreskin.

Adhesions or bridge: Penile adhesions are quite common, especially when foreskin is tight (phimosis) which prevents retraction in the uncircumcised or incompletely circumcised penis. Skin bridging is a complication of early childhood circumcision where scar between original inner ; outer foreskin is fused to head of penis. Suggest you consult a urologist because foreskin should not remain stuck to glans.

my tip of glans turns very big while erection and colour also changed whats the problem? because of it i cant easily open my foreskin?

: more blood flows to the penis causing it to be engorged during erection also causing change in color as compared to flaccid state in some people, which may vary a little.  Not to worry. if you cannot pull back your foreskin only at this time do not worry. But if you cant pull it back at all you should see a Urologist

Normal: Your foreskin must be connected to the based of the glans else it will not be able to protect the rest of your penis from irritation .

It's grown together: Sometimes the foreskin can begin to stick to the glans.  This often starts in childhood.  Gentle stretching and retraction of the foreskin will sometimes help in the younger years, but at your age, it is likely you should see a doctor to determine if stretching is still an option, or if a surgical procedure of clipping the attachments is needed.  This is not a circumcision, but a small clipping.
