Put a 16oz: Bottle of Poland spring in the freezer after it is iced sit down and roll your foot on it, take an antiinflammatory like Motrin and wear a supportive shoe, put an arch support in it which can be bought at local pharmacy. If pain persists see foot doc.

Do not need removal: A plantar calcaneal spur  very seldon need to be removed and only on very special circustances.The heel spur is not the source of the heel pain, the source of the heel pain is due to plantar fasciitis and in rare occasion due to entrapment of a very small nerve(called the nerve of the abdq).Conservative measures work very well the majority of the time.

Yes but...: You'd be better served rolling a frozen water bottle on your heel, stretching your Achilles' tendon, and wearing shoes with good arch support. Take the Motrin if those don't help as directed on the bottle. A 5-day course is what I would do but be careful as that med can irritate your stomach and raise your blood pressure.

Not necessarily: Pain from a calcaneal spur is generally caused by inflammation where the soft tissue (tendon or fascia) attaches to the spur.   You can have a spur in the back of the heel where the achilles' tendon attaches or on the bottom of the heel where the plantar fascia attaches.  You can also have pain in these areas without having a spur, just strain on the soft tissue.

Nothing w/o symptoms: You need to relay your symptoms to a podiatrist (foot doctor) before interpreting any scan / test. It sounds like this X-ray shows some bony growth in the right heel (calcaneal spur) that's not causing any inflammation (reactive change). The trigonum bone in the foot has fluid pockets &amp; some spurs that'd cause pain on the inside of the foot if it were symptomatic. It could be diabetic Charcot foot

Bone abnormalities: Your plantar fascia is the bow string of your foot which helps maintain the arch.  The fascia connect to the inner aspect of your heal .  It is common for this to get pulled and even get avulsed a bit and cause pain. Calcaneal spur is common. Osteochondral dissicans is when a piece of the cartilage that lines the joint breaks off. Looks like a puzzle piece was lost.
