Painful?  Long time?: Could be inflamed taste buds (benign), which are/are not painful.  Could be herpes if painful.  Warm salt water rinses and hydration help reduce inflammation.  Anything on the tongue longer than 3 days see a dentist to make sure nothing serious!

Pain, swelling...: If you're concerned about infection, you really ought to see a dentist for evaluation.  An infected tongue usually has some sort of lesion there (such as a cut/laceration) to introduce the infection and is painful & swollen.  Again, a lot of the diagnosis is based on visual appearance as well, so see a dentist to have it checked out.

Of course: If she is contagious, a fresh wound in the tongue is the perfect portal for admittance of an infectious agent--but then again, so is the gingiva. Of course, if she is infected (with what?) and you are not, why are you performing oral without a dam? If the piercing is reasonably well established, normal, healthy vaginal and anal bacteria will likely not infect the pierced canal.

Yes: It is possible, but i also suggest screen for HIV if you sexually active. If not treated for the infection you mentioned, it can become septic and die. You should see a doctor asap to get a full check up and get treated.

Muscle tie: I have never heard of getting a charlie horse in the tongue.  Tongue muscles exert force when extending the  tongue outward.  "sticking your tongue out" so to speak.  Therefore highly unlikely.

Possibly: Problems like these can only be correctly handled by your doctor in person.  He/she needs to listen to you, perform an examination and possibly run labs or other tests. That's the only way he/she can find out what's going on and what to do about it.
